From the 31st July 2016 all NHS services must provide accessible information in a format that their patients, service users, parents and carers can read.
The Accessible Information Standard has been introduced to ensure that individuals living with disabilities have access to information that they can understand.
Anyone wishing to receive information in an alternative format must contact their GP stating which format is required. In order to make this process simpler the RNIB have produced an “Accessible Format Request Template” which can be downloaded below.
